Unlike traditional accommodation funding, ILO doesn’t cover the cost of the home itself. Instead, it provides a tailored package of supports that are built around your lifestyle, needs, and goals. It’s all about creating a living arrangement that feels right for you.

RiseOn is proud to have a skilled and compassionate clinical team made up of Registered Nurses, Enrolled Nurses, and Clinical Nurses with extensive experience in delivering specialised care to participants with complex health needs. Our nurses work closely with experienced support workers to ensure every aspect of high-intensity care is delivered with precision, professionalism, and empathy.
We are equipped to manage a wide range of complex care needs, including:
Advanced bowel care procedures
· Enteral feeding support (nasogastric or PEG tube management)
· Tracheostomy care and maintenance

Specialist Disability Accommodation (SDA)
Specialist Disability Accommodation is purpose-built housing designed to supportpeople with significant physical or functional impairments, or those with very highsupport needs. These homes are thoughtfully designed to create a safe, accessible,and supportive environment that makes daily living—and accessing care—much easier.
SDA can be a shared home with a small group of residents, or a private residence for individuals who require more personalised living arrangements. The type of home depends on your unique needs and preferences.
SDA funding helps cover the cost of the building itself, ensuring you have access tohigh-quality housing suited to your level of support. While it doesn’t include theday-to-day services or personal support provided within the home, it does offer astable, supportive foundation from which those services can be delivered.
You may still be responsible for regular living expenses such as rent or utilities, butSDA ensures your home environment is aligned with your care needs—supportingyour independence, safety, and overall well-being.
Case Management (Package Management)
Case Management, also known as package management, is more than just coordinating services it’s about building a care journey that truly reflects who you are and what you need.At RiseOn, you’ll be supported by a dedicated Neighbourhood Coordinator who will be with you every step of the way. Their role is to ensure your care is not only well-managed, but also deeply aligned with your goals, values, and lifestyle.
Your Coordinator will: Take the time to understand your goals, needs, and aspirations Respect and incorporate your personal preferences, culture, and daily routine. Identify suitable home care options and develop tailored support strategies. Monitor your progress and adjust supports as your needs evolve. Assist with budgeting, service management, and ongoing administration With RiseOn, case management is about delivering care that adapts to you empowering you to live with greater independence, clarity, and confidence.
